Responsibilities
- Perform comprehensive occupational therapy evaluations for residents in a skilled nursing setting.
- Develop, implement, and monitor individualized treatment plans to improve activities of daily living, mobility, cognition, and independence.
- Provide direct therapeutic interventions and supervise rehabilitation activities as prescribed, adapting plans to progress notes and interdisciplinary input.
- Document progress, outcomes, and any changes in residents’ status in the medical record, ensuring accuracy and compliance with regulatory standards.
- Collaborate with the treatment team, including PTs, speech therapists, nursing staff, and physicians, to coordinate care and support discharge planning and goals.
Requirements
- Active occupational therapy license in New Mexico
- Ability to practice in a skilled nursing facility setting
- Strong patient assessment, documentation, and communication skills
- Ability to work within an interdisciplinary team and adapt to a fast-paced environment
- Willingness to complete credentialing and background checks as required
