Responsibilities
- Perform comprehensive OT evaluations for residents in the skilled nursing setting and develop goals to improve activities of daily living, mobility, cognition, and safety.
- Plan and implement individualized treatment programs, utilizing therapeutic activities, adaptive equipment, and assistive technologies; track progress toward goals.
- Document assessments, treatment plans, and progress notes accurately in the chart and ensure compliance with privacy and regulatory standards.
- Collaborate with an interdisciplinary team (including physical therapy, nursing, social work, and SLP) to support discharge planning and coordinated care.
- Educate residents and families on ADL strategies, safety modifications, home programming, and community reintegration as appropriate.
- Participate in transition or discharge planning to optimize continuity of care and timely return to community or next level of care.
Requirements
- Active Occupational Therapist license in New Hampshire or eligibility for licensure by endorsement.
- Degree from an accredited OT program (OTD or Master's level as applicable).
- Ability to work the scheduled shift (08:00–16:30) and commit to a 15-week travel assignment with 35 guaranteed hours per week.
- Experience in or ability to work effectively in a geriatric rehabilitation/Skilled Nursing Facility environment.
- Must be legally eligible to work in the United States and able to complete standard travel credentialing.
