Responsibilities
- Conduct occupational therapy evaluations and re-evaluations for residents in a skilled nursing facility
- Develop and implement individualized treatment plans to address activities of daily living, mobility, strength, and functional independence
- Provide direct therapy services, monitor progress, adjust plans of care, and document outcomes
- Collaborate with an interdisciplinary team (nursing, physical therapy, speech therapy, social work) to coordinate care and optimize discharge planning
- Educate residents and families on home exercise programs, safety, and adaptive strategies
- Maintain accurate, timely documentation in accordance with facility standards and regulatory requirements
