Job Summary
Occupational Therapist Home Health (OT HH) The organization is seeking an Occupational Therapist for a home health assignment in the northeastern Sacramento area, including Folsom and Woodland, Yolo County. The role involves providing evaluations and revisits, with an average of four visits per day, to support patient recovery and independence in a home setting.

Job Details

• Schedule: Monday through Friday, 8:30 AM to 5:00 PM, with a minimum of 40 hours per week.
• Required Qualifications: Current California Occupational Therapist (OT) license, BLS certification, National Provider Identifier (NPI), and a valid drivers license with proof of automobile insurance. Minimum two years of experience in a healthcare setting, specifically in home health.
• Preferred Qualifications: Experience with home health services and familiarity with the Cerner charting system.
• Additional Information: The assignment includes completing four visits per day, a mix of evaluations and revisits. Mileage reimbursement is available at the GSA rate with prior approval. Coverage area includes north-eastern Sacramento into Folsom and Woodland in Yolo County. The hospital can cancel up to four shifts during the 13-week assignment, or three for shorter-term assignments. A copy of a valid drivers license and proof of automobile insurance are required at submission.
