This position involves providing occupational therapy services within a home health setting, serving patients with diverse diagnoses. The role requires a licensed therapist proficient in OASIS and Homecare Homebase systems, with a focus on patient-centered care and efficient home visits.
Job Details
Schedule: Dayshift, 8-hour shifts, minimum of 40 hours per week, starting on 11/16/2026 through 02/15/2027.
Required Qualifications: Valid occupational therapist license, minimum of 1 year of relevant home health experience, proficiency in OASIS and HCHB (Homecare Homebase), active drivers license, and personal transportation. Must have current BCLS certification.
Preferred Qualifications: Experience with community hospital or home health agencies, familiarity with home health documentation and patient management systems.
Responsibilities: Deliver occupational therapy evaluations and interventions aligned with patient care plans, ensure timely documentation, and coordinate with interdisciplinary teams.
Certifications: Current BCLS certification is required.
Additional Information: The position may require occasional travel within a radius of approximately 60 miles daily, with minimal drive time. Modifiable modules are billable and completed during orientation. Travelers are expected to have their own transportation, and RTO requests must be submitted at application.
