Job Summary Occupational Therapist (OT) Home Health in North Napa Valley. This role involves providing home health occupational therapy services to geriatric patients within a designated territory. The position requires recent experience, valid licensure, and proficiency with Homecare Homebase (HCHB) electronic health record system.
Job Details
Certifications: BLS (Basic Life Support) required.
Education & Experience: Minimum of 1 year of relevant experience in home health. Experience with OASIS documentation required for staff completing Start of Care (SOC). Proficiency in HCHB (Homecare Homebase) required.
Location & Schedule: The position is based in North Napa Valley with a schedule of Monday through Friday, 8-hour shifts, totaling 40 hours per week.
Responsibilities: Provide occupational therapy services to geriatric patients, complete timely documentation, and maintain effective communication.
Additional Information: Must have an active drivers license, access to personal transportation, and be flexible with the territory. RTO (Requested Time Off) must be submitted at the time of application. On-call rotation may be required; discussed during interview.
Submission Requirements: All candidates must be licensed in hand and eligible for placement without prior employment restrictions from previous roles within the last 6 months.
