Make a meaningful difference as a travel OT in Phoenix, AZ. We seek a dedicated Occupational Therapist to provide coordinated, patient-centered care in a home health environment. The clinician will plan, implement, and evaluate individualized care plans to help patients develop, regain, or maintain daily living skills and independence.


Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program.
  • Current, active Arizona Occupational Therapist license required prior to start.
  • Proof of current CPR certification required.
  • Current driver's license, reliable transportation, and vehicle insurance required due to home visits.
  • Minimum two years of experience as an Occupational Therapist in an acute care or rehabilitation setting required.
  • Experience performing home health evaluations, developing Plans of Care (POC), progress notes, and documenting clinical interventions in a home care environment preferred.
  • Ability to fabricate and maintain adaptive equipment and hand splints preferred.
  • Ability to perform clinical assessments of ROM, strength, coordination, functional mobility, ADL performance, and cognitive-perceptual screening preferred.
  • Ability to work a flexible schedule and travel locally within the service area required.
