• Assist patients in improving daily living and functional skills

• Educate patients, families, and caregivers on treatment strategies
• Monitor patient progress and adjust treatment plans as appropriate
• Maintain accurate and timely clinical documentation
• Collaborate with multidisciplinary healthcare teams to enhance patient outcomes
