In this skilled setting, the Occupational Therapist provides comprehensive evaluation and treatment for adult and geriatric patients recovering from illness, surgery, injury, or managing chronic conditions. You will conduct functional assessments, develop individualized treatment plans, and implement interventions focused on improving independence with activities of daily living, functional mobility, cognition, and safety. Typical responsibilities include: Conducting initial and ongoing OT evaluations and documenting findings Designing and modifying treatment plans based on patient progress and goals Training patients and caregivers in adaptive techniques, energy conservation, and use of adaptive equipment Collaborating closely with physical therapy, speech therapy, nursing, and medical providers to coordinate care Participating in care conferences, goal-setting, and discharge planning Maintaining timely, accurate documentation within the EMR system The patient population is primarily adult and geriatric, often with orthopedic, neurological, post-surgical, and medically complex conditions. Caseloads may include both short-term rehab patients focusing on safe return home and long-term residents working toward maximizing function and quality of life. Patient ratios and daily schedules are designed to support meaningful, patient-centered care while meeting reasonable productivity expectations. Shifts are typically daytime hours on weekdays, with potential for occasional weekend or holiday coverage depending on census and facility needs. The environment emphasizes teamwork, open communication, and collaboration, with rehabilitation staff working together to create a supportive, patient-focused culture. As an Occupational Therapist in this setting, you will have the opportunity to refine your clinical skills, mentor and collaborate with other clinicians, and contribute to outcomes that are directly meaningful for patients and their families.
