Travel OT Home Health jobs in Wilmington, DE are available for 13-week contracts, Monday through Friday with 8-hour days and 40 hours expected weekly. You must have a Delaware license and home health experience. You will visit 5 to 6 clients per day and document using Homecare Home Base.
Your responsibilities include performing thorough evaluations of patients’ functional status, developing individualized intervention plans, assessing home environments for hazards, and educating patients and families on rehabilitative care. You will collaborate with a multidisciplinary team and manage your schedule independently
