- Evaluate patients and develop individualized treatment plans
- Provide occupational therapy treatments in an SNF setting
- Help patients improve ADLs, mobility, balance, and coordination
- Educate patients, families, and caregivers on therapy plans
- Maintain accurate documentation and progress reports
- Collaborate with physicians, nursing staff, and rehab teams
- Ensure compliance with facility policies and state regulations
