Travel OT Home Health jobs in Exton, PA are available for 13-week assignments with 8-hour day shifts and an average of 5-6 visits per day. You must have a Pennsylvania license and home health experience. Proficiency with Homecare Homebase EMR is required.
Your responsibilities include evaluating patients in their homes, developing individualized treatment plans, and helping clients regain independence in daily activities like dressing, cooking, and grooming. You will assess home safety, educate patients and caregivers, and document care using Homecare Homebase. You will cover Chester County, providing care in a variety of home environments.
