Requirements:- NBCOT (National Board for Certification in Occupational Therapy) certification required
- Current Occupational Therapist state license
- Valid driver's license with photo identification
- BLS certification and other applicable clinical certifications
- Completion of background check and drug screening
- Current tuberculosis test, Hepatitis B vaccine records, and influenza vaccine or declination
- Skills checklist and documentation of competencies
