Join a dedicated home health team as an Occupational Therapist serving patients in and around Grand Junction, Colorado. As a Home Health Occupational Therapist, you will provide one-on-one, patient-centered care in a variety of home settings across Grand Junction and nearby communities. You will perform comprehensive evaluations, develop individualized plans of care, and implement interventions that help patients regain function, improve safety, and maintain independence in their daily routines. Typical responsibilities include conducting thorough home safety assessments, recommending and training patients in the use of adaptive equipment, developing energy conservation and fall prevention strategies, and educating patients and caregivers on self-management techniques. You will work closely with an interdisciplinary team to coordinate care, communicate changes in patient status, and adjust treatment plans as needed to support optimal outcomes. Your typical day will include traveling between patient homes, completing initial and follow-up assessments, documenting care in an EMR system, and collaborating with team members via phone or secure messaging. Caseloads are designed to be manageable for safe, high-quality patient care, with visit volumes calibrated to allow you to provide thorough, hands-on treatment and education. You can expect to see a range of adult and geriatric patients with conditions such as post-surgical recovery, stroke, cardiac and pulmonary diagnoses, neurological disorders, and general debility or deconditioning. Shifts are generally daytime, Monday through Friday, with some flexibility for patient needs and scheduling preferences. Weekend or holiday coverage may be required on a rotational basis, depending on agency needs. The role involves autonomy and independence in the field, supported by a collaborative clinical and administrative team that is available for guidance, case conferencing, and mentorship. This position is an excellent fit for an Occupational Therapist who enjoys independent practice, problem-solving in real-world home environments, and building long-term therapeutic relationships.
