Occupational Therapist - Home Health - (OT - HH)

This home health Occupational Therapist traveler opportunity is based in the picturesque Santa Ynez Valley of California’s Central Coast, an area known for its rolling vineyards, oak-studded hills, and charming small towns. The region offers a unique blend of peaceful countryside living with convenient access to the amenities of nearby coastal cities such as Santa Barbara. The Santa Ynez Valley is a renowned wine region, home to award-winning vineyards, tasting rooms, and farm-to-table dining. The area’s quaint communities, including Danish-inspired Solvang and historic Santa Ynez, offer a welcoming atmosphere with local boutiques, cafes, and seasonal festivals. Outdoor enthusiasts will appreciate abundance of hiking and biking trails, scenic back roads, and nearby opportunities for horseback riding and exploring the surrounding mountains. The mild Mediterranean climate supports year-round outdoor recreation and a relaxed, healthy lifestyle. As a traveler in this home health OT role, you will provide one-on-one, patient-centered care in the comfort of patients’ homes across the valley and surrounding rural areas. You can expect a caseload primarily consisting of adult and geriatric patients with a range of diagnoses, including post-orthopedic surgery, neurologic conditions, cardiopulmonary disease, deconditioning, and chronic illness management. Typical responsibilities include initial and follow-up assessments, development and progression of individualized care plans, home safety and fall risk evaluations, ADL and IADL training, energy conservation strategies, adaptive equipment and DME recommendations, and caregiver education. A typical day includes driving between patient homes throughout the Santa Ynez Valley, managing a schedule of several visits per day depending on evaluation versus treatment mix. Visit types may include start-of-care assessments, routine treatment visits, recertifications, and discharges. You will collaborate with an interdisciplinary home health team that may include nurses, physical therapists, speech therapists, medical social workers, and home health aides. Communication is generally coordinated via phone, secure messaging, and EMR documentation, with supportive clinical leadership to assist with complex cases. Shifts are generally daytime, Monday through Friday, with some flexibility to accommodate patient and therapist scheduling needs. Weekend or evening work, if needed, is typically limited and planned in advance. Documentation is completed electronically, and you will be expected to manage your time effectively to balance travel, patient care, and timely charting. The home health agency is traveler-friendly and accustomed to integrating contract clinicians into their workflows. You can expect a structured orientation to the service area, EMR system, and documentation standards, along with ongoing support from clinical supervisors. The territory is designed to be manageable, with an effort to cluster visits geographically whenever possible to minimize driving time and maximize patient-facing care. This assignment is an excellent fit for Occupational Therapists who value autonomy, variety in their day, and the opportunity to build meaningful relationships with patients in their home environment.
