Job Description & Requirements
Occupational Therapist - Home Health - (OT - HH)
StartDate: 9/14/2026 Available Shifts: 8 D Pay Rate: $2395.00 - $2468.00

This Home Health Occupational Therapist position is based in the welcoming community of Danbury, Connecticut, a city that blends classic New England charm with modern conveniences. Danbury offers a lively downtown with local shops, restaurants, and cafes, as well as easy access to scenic parks, wooded trails, and nearby Candlewood Lake for year-round outdoor recreation. Situated in western Connecticut, Danbury provides a comfortable suburban lifestyle with quick highway and rail access to New York City and other regional destinations, making it an attractive home base for those who enjoy both a peaceful environment and occasional big-city amenities. In this role, you will provide occupational therapy services to patients in their homes across Danbury and surrounding communities. You will help individuals regain independence after illness, injury, or surgery and support them in remaining safely at home. Your typical day will include traveling to patient homes, performing comprehensive evaluations, collaborating with the broader home health team, and delivering one-on-one, functionally focused treatments tailored to each patient’s living environment. Core responsibilities include completing detailed functional assessments, including home safety evaluations and analysis of activities of daily living and instrumental activities of daily living. You will develop and implement individualized plans of care, focusing on maximizing independence in self-care, mobility within the home, household tasks, and community participation. You will recommend and train patients and caregivers in the use of adaptive equipment and durable medical equipment, instruct on energy conservation and joint protection strategies, and provide education to reduce fall risk and promote safe movement. You will work closely with nursing, physical therapy, social work, and other disciplines to coordinate services, update care plans, and monitor patient progress. Typical caseloads are managed to allow for meaningful one-on-one time in the home, with visit lengths designed to support thorough assessment, intervention, and patient and caregiver education. Patient populations commonly include older adults recovering from joint replacements or fractures, individuals managing cardiac or pulmonary conditions, patients after hospital or skilled nursing facility discharge, and those living with neurologic conditions such as stroke or Parkinson’s disease. Home health practice in the Danbury area offers a strong opportunity to build clinical skills and professional versatility. You will gain experience adapting interventions to a wide range of home environments, troubleshooting real-world barriers to independence, and partnering directly with families and support networks. The role emphasizes professional autonomy, as you manage your daily schedule, plan visit routes within your territory, and make clinical decisions in collaboration with the wider care team. At the same time, you will have support from supervisors and colleagues who value communication, shared problem-solving, and a patient-first approach. Working in Danbury offers the advantage of serving a diverse, growing population while enjoying an attractive quality of life. The area features reputable schools, community events, and local arts and cultural activities. Outdoor enthusiasts appreciate nearby lakes, hiking trails, and parks, and the region’s four distinct seasons highlight everything from vibrant fall foliage to summer waterfront activities. This combination of professional opportunity and lifestyle benefits makes a home health OT role in Danbury especially appealing for clinicians who value both meaningful, independent practice and a comfortable, engaging place to live and work.
