Communication with referring providers and participation in care conferences will help shape and refine each patient’s plan of care. Documentation will be completed in an electronic medical record, with an emphasis on accuracy, timeliness, and compliance with regulatory standards. Patient caseload typically consists of adult and geriatric patients with a range of conditions, such as post-surgical recovery, neurological diagnoses, chronic disease management, and general decline in functional status. Visit volume and scheduling are structured to support thorough, individualized care while maintaining reasonable travel demands within the Pleasant Hill area and nearby communities. Shifts generally follow daytime hours, with scheduling adapted to patient needs and agency guidelines.
