This Home Health Occupational Therapist position is based in Pleasant Hill, California, a welcoming community in the East Bay known for its relaxed pace of life, attractive suburban neighborhoods, and strong sense of community. You will address areas such as self-care, home management tasks, adaptive equipment use, cognitive strategies, and energy conservation. Home safety evaluations, recommendations for environmental modifications, and coordination of assistive devices will be a key component of your work. You will collaborate closely with an interdisciplinary team that may include nursing, physical therapy, speech therapy, social work, and home health aides, ensuring coordinated and comprehensive care. Communication with referring providers and participation in care conferences will help shape and refine each patient’s plan of care. Documentation will be completed in an electronic medical record, with an emphasis on accuracy, timeliness, and compliance with regulatory standards. Patient caseload typically consists of adult and geriatric patients with a range of conditions, such as post-surgical recovery, neurological diagnoses, chronic disease management, and general decline in functional status. Visit volume and scheduling are structured to support thorough, individualized care while maintaining reasonable travel demands within the Pleasant Hill area and nearby communities. Shifts generally follow daytime hours, with scheduling adapted to patient needs and agency guidelines.
