Travel Occupational Therapist (OT) - $2,688 to $2,888 per week in Danbury, CT

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Occupational Therapy from an accredited educational program.
• Active Connecticut Occupational Therapy license required to start the assignment.
• Minimum 1 year of home health experience required.
• BLS certification required or may be required from AHA or ARC.
• Experience providing OT services focused on upper extremity function, activities of daily living, and cognitive-perceptual motor function preferred.
• Experience with home health documentation platforms such as Home Care Home Base (HCHB) preferred.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0700-1530.
• Hours per week: 40.
• Primary caseload: post-orthopedic surgery, heart failure, and COPD patients with emphasis on energy conservation and upper extremity ADL training.
• Supervisory responsibilities: may supervise COTA(s) and home health aides as appropriate.
• Must provide direct occupational therapy services in accordance with physician-established plan of care and maintain visit records including evaluations and re-evaluations.
• Travel assignment duration: 13-week travel assignment.
