In this position, you will provide one-on-one Occupational Therapy services to patients in their homes, helping them regain or maintain independence with daily activities after illness, injury, or surgery. Your work will focus on functional assessments, home safety evaluations, and individualized treatment plans aimed at promoting safety, mobility, and quality of life in the home environment.
Typical responsibilities include performing comprehensive OT evaluations in the home; designing and implementing personalized treatment plans; recommending adaptive equipment and home modifications; educating patients and caregivers on safe techniques, energy conservation, and fall prevention; and coordinating closely with nursing, physical therapy, speech therapy, and medical providers. You will also update the care team on patient progress and adjust interventions based on clinical findings and patient goals.
A typical day may include visiting several patients in Danbury and nearby communities, with scheduling designed to balance productivity with manageable travel time. You will have the autonomy to structure your route and visits while adhering to agency standards and regulatory guidelines. Documentation is completed electronically through an EMR system, often supported by mobile tools to streamline charting and communication.
The caseload primarily includes adult and geriatric patients, many with chronic conditions, post-acute needs following hospitalization or surgery, or functional changes related to neurologic or orthopedic diagnoses. Patient volumes and visit expectations are set to allow for thorough, hands-on care and meaningful education during each home visit.
