Job Title: Occupational Therapist Profession: Therapist - Occupational Specialty: Occupational Duration: 13 weeks Shift: Day Hours per Shift: 8.5 Experience: Minimum 1 year of home care experience License: Licensed Occupational Therapist in the state of Connecticut Certifications: BLS preferred Must-Have: - Home care experience - Experience in working with heart failure and COPD patients Description: Provides services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain. Emphasis is placed on upper extremity function, maximal independence in activities of daily living, and cognitive perceptual motor function. Varied patient caseload includes post-orthopedic surgery patients and heart failure and COPD patients for energy conservation. Provides direct occupational therapy services in accordance with the plan of care established by the physician. Maintains records of visits to clients, including evaluations, plans for care, and clients' responses to therapy. Provides education and counseling to patients and family members regarding the occupational therapy treatment regimen. Re-evaluates client progress and treatment regimen, making changes as indicated and under the physician's recommendations. Supervises care provided by certified occupational therapy assistants and home health aides as appropriate. Works with occupational therapy as the main therapy for heart failure and COPD patients focusing on energy conservation.
