About PACE
PACE (Program of All-Inclusive Care for the Elderly) is a federal and state-funded healthcare program that provides comprehensive and coordinated care for seniors, helping them remain safely in their homes and communities.
Key Responsibilities
- Conduct occupational therapy evaluations and assessments for participants.
- Develop and implement individualized treatment plans based on patient needs and goals.
- Assist patients in improving mobility, strength, coordination, and daily living skills.
- Recommend adaptive equipment and therapeutic interventions to enhance independence and safety.
- Collaborate with physicians, nurses, caregivers, and interdisciplinary team members regarding patient care plans.
- Monitor patient progress and modify treatment plans as necessary.
- Educate patients and caregivers on therapy techniques, safety measures, and home exercise programs.
- Maintain accurate and timely patient documentation in compliance with company and regulatory standards.
- Participate in care conferences and interdisciplinary team meetings.
