Key Responsibilities- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop individualized plans of care using standardized assessments and evidence-based practice.
- Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Monitor and document patient progress, adjust treatment plans accordingly, and evaluate outcomes for effectiveness in the electronic documentation system.
- Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders and discharge planning.
- Participate in required meetings such as patient care conferences, utilization review, and rehabilitation meetings, as well as facility and company required trainings.
- Supervise and support occupational therapist assistants, rehabilitation aides, and/or students as applicable.
Required Qualifications- Active Occupational Therapist (OT) license in the state of Oklahoma required at time of submission.
- Minimum of 1 year of OT experience required.
- Long Term Care (LTC) or rehabilitation setting experience required.
- Adherence to NBCOT and state-required practice acts, code of conduct, and ethics.
- Ability to accurately record treatment minutes and maintain patient records per all regulatory and clinical requirements.
Preferred Experience / Skills- New graduates with completed Level II fieldwork experience (not solely Level I observation) will be considered.
- Experience supervising occupational therapist assistants or rehabilitation aides.
