Assess patients' level of functioning upon physician orders and develop individualized treatment plans based on evaluation results and patient and family goals.
Plan, implement, and document occupational therapy treatments in accordance with applicable federal, state, and local standards, regulations, and guidelines.
Participate in the interdisciplinary team process, including team conferences, and collaborate with other team members to coordinate patient care.
Manage discharge planning for each patient, including placement recommendations, patient and family education, and adaptive equipment needs.
Supervise Certified Occupational Therapy Assistants (COTAs), OT and COTA students, and assist with new staff orientation.
Promote patient and employee safety through safe patient handling, infection control practices, and adherence to all safety policies and procedures.
Required Qualifications
Current and valid Occupational Therapist licensure or eligibility to practice in the state of Kentucky.
Demonstrated ability to perform comprehensive patient evaluations and develop appropriate, individualized treatment plans.
Knowledge of and adherence to federal, state, and local occupational therapy standards and regulations.
Ability to complete thorough and timely clinical documentation including daily progress notes, treatment plans, and discharge summaries.
Strong communication skills with the ability to interact effectively with patients, families, and interdisciplinary team members.
Preferred Experience / Skills
Prior experience supervising COTAs, therapy students, or other clinical staff.
Experience with discharge planning processes including equipment recommendations and caregiver training.
Background in a rehabilitation or post-acute care setting.
