Key Responsibilities

• Evaluate and treat patients within a Skilled Nursing Facility (SNF) setting to restore or improve functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
• Educate patients and family members on therapeutic techniques and home exercise programs

Required Qualifications

• Graduate of an accredited Occupational Therapy program
• Current and valid Occupational Therapist (OT) licensure in the state of California
• Current CPR/BLS certification
• Experience working in a Skilled Nursing Facility (SNF) setting

Preferred Experience / Skills

• Prior experience with geriatric patient populations
• Familiarity with MDS documentation and Medicare guidelines in a SNF environment
