Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore or maintain functional independence
• Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care team members to coordinate patient care
• Educate patients and caregivers on therapeutic techniques and home programs to support continued progress

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Colorado
• Graduate of an accredited Occupational Therapy program
• Current CPR/BLS certification
• Experience working in a skilled nursing facility or similar post-acute care environment

Preferred Experience / Skills

• Prior experience in a SNF or long-term care setting
• Familiarity with MDS documentation and Medicare guidelines
• Strong communication and team collaboration skills
