Key Responsibilities- Evaluate patients in a home health setting to assess functional abilities and develop individualized treatment plans
- Provide therapeutic interventions aimed at improving patients' ability to perform activities of daily living
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care
- Educate patients and caregivers on therapeutic techniques and home safety strategies
Required Qualifications- Current and valid Occupational Therapist licensure in the state of Iowa
- Graduate of an accredited Occupational Therapy program
- Current NBCOT certification
- Experience working in a home health or similar clinical setting
- Strong documentation and communication skills
Preferred Experience / Skills- Prior home health experience with a diverse patient population
- Familiarity with home health electronic medical record systems
- Strong ability to work independently and manage a patient caseload in a community-based environment
