Schedule / Job Type

Contract | Days | Monday–Friday, 8:00 AM–4:30 PM with weekend rotation; must be available to work Thanksgiving and New Year's

Key Responsibilities

• Evaluate patients' functional status and implement the physician's plan of care, including assessment and reassessment using objective tests and measurements
• Provide therapeutic services designed to help patients regain optimal function in daily activities including self-care, leisure, and work
• Communicate observations, findings, and treatment recommendations as part of an interdisciplinary team through individual interactions and team meetings
• Provide education and direction to patients and coworkers as appropriate
• Complete accurate and concise documentation of all communications, coordination, and treatment activity
• Adapt to emergency situations and remain alert to changes in patient condition while recognizing and protecting patient rights

Required Qualifications

• Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Eligible for or holds the National Registration Examination of the American Occupational Therapy Association
• Active Oregon State Occupational Therapist license required
• Current BLS certification required
• Valid driver's license and personal vehicle required — patient care is provided in patients' homes
• Proof of current auto insurance required

Preferred Experience / Skills

• Registration as an Occupational Therapist with the American Occupational Therapy Association
• Continuing education relevant to skill level and area of practice
• Current knowledge of occupational therapy as it relates to physical disability, rehabilitation, mental health, the elderly population, long-term care, and home health and hospice
• CRASE training (may be required if assignment extends beyond six months)
