Key Responsibilities- Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
- Implement therapeutic interventions to improve patients' functional independence and quality of life
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications- Active Occupational Therapist (OT) license in the state of Ohio or eligibility to obtain
- Graduate of an accredited Occupational Therapy program
- Current NBCOT certification
- Experience working in a skilled nursing facility (SNF) setting
Preferred Experience / Skills- Prior travel or contract therapy experience
- Familiarity with MDS documentation and PDPM reimbursement models
