Key Responsibilities- Provide occupational therapy services across multiple facility locations on a rotating schedule
- Deliver patient care on a split-site basis, covering various locations throughout the week
- Evaluate and treat patients in accordance with occupational therapy standards of practice
- Maintain accurate and timely clinical documentation for all patient encounters
- Collaborate with interdisciplinary care teams to support patient goals and outcomes
Required Qualifications- Active Occupational Therapist (OT) license in the state of Illinois
- Graduate of an accredited Occupational Therapy program
- Current NBCOT certification
- Ability to travel to and work across multiple facility locations as assigned
Preferred Experience / Skills- Prior experience working in a skilled nursing or long-term care setting
- Experience managing a caseload across multiple sites simultaneously
