Key Responsibilities

• Provide occupational therapy services to patients across a multi-county home health territory
• Meet a minimum productivity expectation of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day
• Utilize HCHB (Homecare Homebase) for documentation and care coordination
• Deliver patient-centered care within assigned home health territory spanning multiple counties

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Washington
• Minimum of one year of recent home health OT experience
• Proficiency with HCHB (Homecare Homebase) documentation platform
• Ability to travel and provide services across a large multi-county territory

Preferred Experience / Skills

• Familiarity with home health operations across diverse geographic service areas
• Strong time management skills to meet daily documentation deadlines
• Experience managing a high-volume caseload in a home health setting
