Occupational Therapist – Home Health (OT)

Schedule / Job TypeContract | 13-Week Assignment | Full-Time | Days | Monday – Friday, 40 hours per week | Onsite
Key Responsibilities- Provide occupational therapy services to patients in a home health setting within an assigned territory
- Conduct evaluations and develop individualized treatment plans to address patients' functional needs
- Deliver skilled occupational therapy interventions aimed at improving patients' independence and quality of life
- Document patient progress, treatment plans, and clinical notes in accordance with home health standards
- Coordinate care with interdisciplinary team members to ensure comprehensive patient outcomes
Required Qualifications- Active and valid Occupational Therapist (OT) license in the state of Wisconsin
- Minimum of 12 months of home health experience within the past 3 years
- Strong clinical documentation and communication skills
- Ability to work independently in a home health environment
Preferred Experience / Skills- Experience with HCHB (Homecare Homebase) electronic medical records system
- Familiarity with home health regulatory and compliance requirements
