    Key Responsibilities
    • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders; develop and implement individualized plans of care in accordance with regulatory and clinical practice standards.
    • Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
    • Assess and adjust patient treatment plans based on progress; evaluate treatment outcomes and modify services accordingly, including appropriate referrals.
    • Document all required clinical and regulatory information in the electronic documentation system, accurately recording treatment minutes per applicable requirements.
    • Communicate effectively with supervisors and interdisciplinary health team members regarding patient progress, barriers, and treatment plans; provide comprehensive discharge summaries.
    • Supervise and support occupational therapist assistants, rehabilitation aides, and/or students; participate in patient care conferences, utilization review meetings, and required trainings.
    Required Qualifications
    • Active Occupational Therapist (OT) license in the state of Oklahoma (required at time of submission).
    • Minimum of 1 year of OT experience, or new graduate with completed Level II fieldwork experience.
    • Long Term Care (LTC) or rehabilitation setting experience.
    • Adherence to NBCOT and state-required practice acts, code of conduct, and ethics.
    • Ability to obtain necessary DME or supplies per regulatory and clinical practice requirements.
    Preferred Experience / Skills
    • Experience utilizing standardized assessments and evidence-based practice to guide clinical interventions.
    • Prior experience providing consultation and counseling to patients, families, caregivers, and other service providers.
