Key Responsibilities- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders; develop and implement appropriate plans of care in accordance with regulatory and clinical practice standards.
- Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assess and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes for effectiveness in the electronic documentation system.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical and cognitive disorders.
- Participate in required meetings such as patient care conferences, utilization review meetings, and rehabilitation meetings; complete comprehensive discharge summaries per regulatory requirements.
- Supervise and support occupational therapist assistants, rehabilitation aides, and/or students as needed.
Required Qualifications- Active Occupational Therapist (OT) license in the state of Oklahoma required at time of submission.
- Minimum of 1 year of OT experience required; new graduates with Level II fieldwork experience will be considered.
- Long Term Care (LTC) or rehabilitation setting experience required.
- Adherence to NBCOT and state-required practice acts, code of conduct, and ethics standards.
- Ability to accurately record treatment minutes and maintain patient records per all regulatory and clinical requirements.
Preferred Experience / Skills- Experience utilizing standardized assessments and evidence-based practice to guide clinical interventions.
- Familiarity with electronic documentation systems used in long term care or rehabilitation environments.
- Experience obtaining DME or supplies per regulatory and clinical practice requirements.
