Contract | Full-Time | Days | Monday–Friday with one weekend per month rotation | Onsite/Field-Based

Key Responsibilities

• Prioritize and conduct start of care visits with next-day mandatory reporting
• Supervise Occupational Therapy Assistants (OTAs) within the assigned territory
• Complete clinical notes and all required documentation within 48 hours of each visit
• Maintain timely communication with supervisor, intake team, and office staff during normal business hours
• Serve patients throughout Alameda County, with the expectation to expand territory as needed
• Meet a productivity requirement of 30 points per week, with caseloads and visit mixes expected to average 8 hours per day and 40 hours per week

Required Qualifications

• Active California Occupational Therapist (OT) License
• Minimum of one year of Home Health experience
• Ability to work Monday–Friday with one weekend per month rotation
• Proficiency or willingness to learn the Kinnser documentation system
• Overtime requires pre-approval by the Clinical Supervisor

Preferred Experience / Skills

• Experience with OASIS documentation including Start of Care, Recertification, Resumption of Care, and Discharge assessments
• Familiarity with point-based productivity models in a home health setting
• Strong time management skills with the ability to manage a varied daily caseload independently
