Key Responsibilities

• Provide occupational therapy services to patients in a home health setting
• Complete OASIS assessments accurately and in accordance with regulatory requirements
• Develop and implement individualized treatment plans to support patient recovery and functional independence
• Travel to patient homes within the service area, with typical daily mileage of approximately 35–50 miles
• Participate in a non-billable orientation period of approximately 8 hours
• Adhere to facility dress code requirements (Olive Green Scrubs)

Required Qualifications

• Minimum 2 years of current Occupational Therapist experience
• Minimum 2 years of current Home Health experience
• Demonstrated OASIS experience
• Active Michigan Occupational Therapist license (LARA)
• Active American Heart Association (AHA) BLS certification
• Personal reliable vehicle, clean driving record, and proof of valid auto insurance

Preferred Experience / Skills

• Experience working in a multi-county or broad geographic home health service area
• Familiarity with home health documentation systems and compliance standards
