Assess patients' level of functioning upon physician orders and develop individualized treatment plans based on evaluation results and patient/family goals
Plan, implement, and document occupational therapy treatments in accordance with applicable federal, state, and local standards, regulations, and guidelines
Participate in the interdisciplinary team process, including team conferences and collaborative care planning
Coordinate discharge planning for each patient, including placement recommendations, patient/family education, and adaptive equipment needs
Supervise Certified Occupational Therapy Assistants (COTAs) and OT/COTA students in the application of occupational therapy theory and techniques
Promote patient and employee safety through safe patient handling, infection control practices, and adherence to all facility safety policies and procedures
Required Qualifications
Active Occupational Therapist (OT) licensure in the state of Indiana or eligibility to obtain
Ability to assess patients, develop treatment plans, and complete all required clinical documentation within defined timeframes
Demonstrated ability to supervise COTAs and students competently
Strong commitment to patient safety, confidentiality, and quality improvement
Preferred Experience / Skills
Experience in an interdisciplinary rehabilitation or acute care setting
Familiarity with discharge planning processes, including adaptive equipment recommendations and caregiver training
Experience mentoring or orienting new staff or students
