Key Responsibilities- Provide occupational therapy services to patients in a home health setting
- Conduct evaluations and develop individualized treatment plans to address functional goals
- Deliver skilled therapeutic interventions to improve patients' daily living activities and independence
- Document patient progress and clinical notes accurately and in a timely manner
- Collaborate with interdisciplinary care team members to coordinate patient care
Required Qualifications- Active Occupational Therapist (OT) licensure in the state of Oregon
- Experience working in a home health setting
- Proficiency with HomeCare HomeBase (HCHB) electronic documentation system
- Ability to work independently in a field-based environment
Preferred Experience / Skills- Prior experience with leave of absence coverage or short-term contract assignments
- Strong time management and organizational skills in a home health environment
