Contract | Days | Monday–Friday, 8:00 AM–4:00 PM | 36 guaranteed hours per week

Key Responsibilities

• Provide occupational therapy services across two care settings: approximately 50% Home Health and 50% Skilled Nursing Facility, with flexibility based on area need
• Evaluate patients and develop individualized treatment plans to improve functional independence and quality of life
• Document patient progress and maintain accurate clinical records using HCHB (Home Health) and NetHealth (SNF) software platforms
• Meet productivity expectations of 90% in the Home Health setting and 85% in the Skilled Nursing Facility setting
• Collaborate with an on-site therapy supervisor and interdisciplinary care team to ensure coordinated patient care

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Minnesota (or eligibility to obtain)
• Ability to work across both Home Health and Skilled Nursing Facility settings as needed
• Proficiency or willingness to learn HCHB and NetHealth documentation systems
• Must reside within 80 miles of the facility to be considered for local placement

Preferred Experience / Skills

• Prior experience in a Home Health or Skilled Nursing Facility setting
• Familiarity with productivity-based therapy models in post-acute care environments
• Strong time management and organizational skills to manage a split-setting caseload
