Key Responsibilities

• Conduct an average of four home health visits per day, including both evaluations and revisits
• Provide occupational therapy services to patients in a home health setting across the assigned coverage area
• Deliver care throughout the north-eastern Sacramento into Folsom area and the Woodland/Yolo County service area
• Complete accurate and timely documentation in accordance with home health standards

Required Qualifications

• Active California Occupational Therapist (OT) license
• Minimum of 2 years of experience in a healthcare setting
• Current Basic Life Support (BLS) certification
• Active National Provider Identifier (NPI) number
• Valid driver's license and proof of current automobile insurance

Preferred Experience / Skills

• Prior home health experience strongly preferred
• Certified Occupational Therapy Assistant (COTA) with home health experience may be considered
