Occupational Therapist – Home Health

Schedule / Job TypeContract | Days | Monday through Friday with one weekend day on-call rotation | 5x8-Hour Shifts (8:00 AM – 5:00 PM)
Key Responsibilities- Provide occupational therapy services to patients in a home health setting
- Complete a minimum of 25 patient visits per week to fulfill guaranteed hours requirement
- Complete timely documentation in accordance with home health standards, including OASIS assessments
- Manage an assigned patient territory with personal transportation between visits
- Participate in on-call rotation as required
- Complete required orientation modules during onboarding
Required Qualifications- Active California Occupational Therapist license (candidates with pending CA license who have applied at time of submission will be considered)
- BLS certification through the American Heart Association (AHA) required
- Active, valid driver's license and personal transportation required
- OASIS experience required
- Timely communication and documentation are mandatory
- Completion of Home Health Checklist is mandatory
Preferred Experience / Skills- Minimum of 1 year of home health experience preferred; candidates without home health experience will be considered
- Familiarity with home health compliance and documentation standards
