Job Overview:
We are seeking a dedicated and compassionate Home Health Occupational Therapist (OT) to join our team. This role is perfect for professionals who enjoy working independently while delivering high-quality, patient-centered care in a home health setting.
Assignment Details:
- Schedule: Monday – Friday
- Shift: 8:00 AM – 5:00 PM
- Weekend Rotation: Required (rotating weekends)
Key Responsibilities:
- Evaluate and treat patients in their homes to improve daily living and functional skills
- Develop and implement individualized treatment plans
- Educate patients and families on therapy techniques and adaptive strategies
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely documentation
Requirements:
- Minimum 1 year of experience as an Occupational Therapist
- Prior Home Health experience required
- Active Illinois OT License
- BLS Certification (AHA preferred)
- Strong communication and organizational skills
- Ability to work independently and manage caseload effectively
