Occupational Therapist – Home Health (Travel Contract)

Schedule / Job TypeContract | Days | Full-Time | Onsite/Field | Shifts available: Tuesday–Saturday, Wednesday–Sunday, or Monday–Friday (5x8-hour shifts, 8:00–17:00)
Key Responsibilities- Provide occupational therapy services to patients in a home health setting across an assigned geographic territory
- Complete OASIS assessments accurately and in a timely manner
- Maintain timely clinical documentation in compliance with agency and regulatory standards
- Manage a caseload requiring completion of 25 or more visits per week to meet guaranteed hours
- Travel independently between patient homes within the assigned coverage territory using personal transportation
- Participate in on-call rotation as required and adhere to all home health compliance requirements including completion of mandatory modules during orientation
Required Qualifications- Active California Occupational Therapist license (pending CA license accepted if application is submitted at time of submission)
- BLS certification through the American Heart Association (AHA) required
- OASIS experience required
- Active, valid driver's license and reliable personal transportation
- Ability to maintain timely communication and documentation
- Completion of Home Health Checklist is mandatory
Preferred Experience / Skills- Minimum of 1 year of home health experience strongly preferred; candidates without home health experience will be considered
- Familiarity with home health compliance standards and field-based caseload management
