Contract (13 Weeks) | Full-Time | Onsite | Days | Monday – Friday, 8:00 AM – 4:30 PM

Key Responsibilities

• Provide skilled occupational therapy services to residents in a skilled nursing facility (SNF) setting
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Implement therapeutic interventions to improve patients' daily living skills and independence
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care team members to ensure comprehensive patient care

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Arizona
• Graduate of an accredited Occupational Therapy program
• Current NBCOT certification
• Experience working in a skilled nursing facility (SNF) or similar clinical setting

Preferred Experience / Skills

• Prior experience in a SNF environment with a geriatric population
• Familiarity with MDS documentation and Medicare/Medicaid guidelines
