Job Title

Contract | Full-Time | Onsite | Day Shift (8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM)

Key Responsibilities
  • Complete comprehensive patient assessments including biopsychosocial, financial, medical, and physical factors to identify discharge planning needs within departmental timeframes
  • Develop and implement individualized discharge plans in collaboration with a multidisciplinary team, ensuring safe transitions and coordination with payors, community resources, and post-hospital care providers
  • Provide short-term counseling including crisis intervention, bereavement support, and adjustment counseling related to medical diagnosis and/or hospitalization
  • Advocate on behalf of patients with referral and payor sources to facilitate access to needed services and community-based resources
  • Perform ongoing reassessments and update care plans to reflect changes in the patient's medical or social condition
  • Ensure compliance with medical/legal reporting requirements including child abuse, adult abuse, and sexual abuse mandates
Required Qualifications
  • Active Licensed Master of Social Work (LMSW) license issued by New York State
  • Minimum of 2 years of social work experience
  • Experience working in a hospital or acute care setting
  • Demonstrated knowledge of community-based resources and discharge planning processes
  • Ability to handle crisis situations and manage competing priorities effectively
Preferred Experience / Skills
  • Experience with complex psychosocial discharge planning cases
  • Familiarity with multidisciplinary care team collaboration and payor coordination
  • Background in quality improvement initiatives within a clinical setting
