Contract | Days | Monday–Friday, 8:30 AM–5:00 PM | One weekend per month required for visits | One major and one minor holiday required | No on-call rotation
Key Responsibilities
Provide occupational therapy services to home health patients across the east Lake County coverage area
Conduct an average of 4–6 patient visits per day
Travel to patient homes within the assigned coverage area, with mileage reimbursement provided per IRS rate
Participate in one weekend per month to support orthopedic cases and scheduled holiday coverage as required
Adhere to the facility's internal call-off policy, including potential rescheduling of weekend or holiday shifts if a call-off occurs
Required Qualifications
Active Florida Occupational Therapist (OT) license required
Minimum of 1 year of occupational therapy experience
Valid driver's license and current auto insurance required
Must be able to provide at least 1 professional reference from entire work history
SSN and date of birth required for credentialing
Preferred Experience / Skills
Prior home health experience
Experience with orthopedic patient populations
