- Evaluate residents functional abilities (ADLs, mobility, cognition, psychosocial factors). - Develop and implement individualized occupational therapy treatment plans. - Provide direct occupational therapy services in compliance with regulations and facility policies. - Monitor, document, and adjust resident progress and treatment plans as necessary. - Collaborate with interdisciplinary team members in care planning and delivery. - Educate residents, families, and caregivers on safety and independence strategies. - Recommend and instruct on adaptive equipment and assistive devices. - Supervise and guide Certified Occupational Therapy Assistants (COTAs) and therapy aides. - Ensure timely and accurate documentation in electronic medical records. - Maintain state-required licensure and continuing education. - Support facility quality improvement and adhere to CMS/facility guidelines. - Work in a skilled nursing/long-term care environment, requiring patient handling. - Contribute to a collaborative, team-oriented, and fast-paced work setting.
