We are seeking a dedicated Occupational Therapist (OT) for a 13-week contract in a Home-Based Primary Care setting in Rockford, Illinois. This role involves providing patient-centered care in a home health environment, helping patients improve daily living and functional independence.
Responsibilities:- Evaluate patients and develop individualized treatment plans
- Provide therapy to improve daily living and functional skills
- Educate patients and families on adaptive techniques and equipment
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely documentation
- Travel to patient homes and deliver quality care
- Collaborate with physicians and healthcare teams
