Patient Care Coordinator (Outpatient Transition Care)

Job Description

We are seeking a dedicated Patient Care Coordinator to focus on patient education, assist with procedures, and provide inpatient transition care at our outpatient setting. The role requires significant interaction with patients as you facilitate their transition and ensure they receive the treatment needed for their ongoing care.

Schedule

This position is scheduled for 5 shifts per week, each lasting 7.5 hours, from 08:00 AM to 04:30 PM, totaling 38 hours per week.

Responsibilities

• Conduct patient education sessions with a focus on treatment and care continuity.
• Assist healthcare providers with various procedures as necessary.
• Manage inpatient transition care to ensure effective outpatient treatment.
• Maintain a patient-to-care coordinator ratio of 1:10.
• Deliver high-quality patient care based on established protocols and guidelines.
• Cultivate a supportive and informative environment for patients and their families.

Required Experience / Certifications / Licensure

• A minimum of 2-3 years of relevant experience in a healthcare setting.
• Previous travel experience is preferred.
• Certification in Basic Life Support (BLS) is required.
