Description

American Traveler is hiring an Occupational Therapist for a home health position requiring 1+ year of experience and a valid driver's license for home visits.

Details

• Home health setting providing skilled visits under Medicare, Medicare Advantage, and commercial insurance
• Diverse patient population including med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, and amputee cases
• 6-8 home visits per day (minimum of 6 points daily, may increase per manager discretion)
• Day shift schedule, 5x8 hour shifts from 8:00 AM to 5:00 PM
• 1-2 weekends required per contract
• Self-scheduled visits and hours based on office needs
• Provided equipment includes laptop, MiFi for internet access, and portable printer
• Documentation completed via BayCare Forms, BayCare Scheduling, and Ace Timesheet systems

Requirements

• Active Florida or Compact OT license required if applicable
• Minimum 1 year of experience required (2 years preferred)
• Must function as an independent practitioner
• BLS certification required
• OASIS training preferred
• LVST certification preferred
• Valid driver's license and reliable vehicle required for home visits
• Cerner charting experience preferred
• Experience with prosthetics, orthotics, and splinting techniques (above/below knee prosthetics, dynamic and static splints)
• Skilled in ADLs, adaptive equipment, home assessments, and wheelchair evaluations
• Experience with CVA rehabilitation, orthopedic conditions, and neurological disorders preferred

Additional Information

• Conduct home assessments, pre-discharge planning, and community re-entry evaluations
• Provide patient and family education on home exercise programs and safety/injury prevention
• Perform evaluations, reassessments, OASIS documentation, and care plan development
• Must be COVID vaccinated or have an exemption form on file
• Flu shot required or declination form with mandatory mask wearing at all times
• Any solid color scrubs permitted
• Willingness to float within the assigned facility or nearby affiliate hospital may be required
• Former BayCare employees must wait 12 months before returning as a traveler
• Candidates must reside more than 100 miles from the facility to be eligible for the travel rate
• RTO is not permitted under standard travel contracts
