Outpatient Physical Therapist (PT OP) The organization is seeking a dedicated outpatient physical therapist to provide patient evaluations, develop treatment plans, and facilitate recovery in a clinical outpatient setting. This role involves educating patients and their families, collaborating with the healthcare team, and ensuring safety and compliance throughout the patient care process.

Job Details

• Position Description: Evaluates assigned patients, develops, implements, and modifies treatment plans aligned with patient needs and physician directives. Responsible for delegation and supervision of therapy services, designing functional treatment programs, and serving as an educator for patients, families, students, and staff. Ensures compliance with organizational safety and confidentiality standards.
• Required Qualifications: Degree in Physical Therapy from an accredited school; current CPR certification; active and applicable license for the state where work will be performed; 0 years of outpatient clinical experience; scheduled to start on 10/12/2026; prior successful assignment with the agency or organization is highly desired.
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Additional Information: Completes comprehensive patient evaluations, develops individualized treatment plans focusing on patient discharge goals, documents in medical records accurately and timely, and participates in professional development activities. Adheres to state practice acts and regulatory requirements, maintains licensure, and contributes to organizational performance improvement initiatives. Demonstrates safety awareness in all clinical activities, including patient and staff safety protocols.
